Abstract
Objective:
The purpose of this paper is to explore the efficacy of utilizing Glucagon-Like Peptide-1 Receptor Agonists (GLP-1RA) to prevent major adverse limb events (MALE) in patients with peripheral artery disease (PAD) versus standard treatment.
Methods:
A narrative literature review was conducted using systematic searches of MEDLINE and Google Scholar for studies evaluating GLP-1RAs and vascular outcomes. English-language, full-text studies published since 2020 assessing GLP-1RA use and limb-related outcomes were included; older studies and those not involving GLP-1RAs were excluded. Primary outcomes include a reduction in MALE and a decrease in risk factors associated with MALE. Secondary outcomes include transcutaneous oxygen pressure (TcPO2), circulating endothelial progenitor cells, vascular endothelial growth factor-a, and 6-minute walking test (6MWT).
Results:
Evidence demonstrated that GLP-1RAs reduce MALE in patients with PAD. Compared with standard treatment, GLP-1RAs showed a reduction in amputations compared to placebo (HR 0.65, 95% CI 0.45-0.95; p=0.03), 2.59 vs 4.33 events per 1000 person-years compared to DPP-4 inhibitors, and 3.6 vs 4.5 events per 1000 person-years compared to SGLT2. Also, a 10% increase in TcPO2 in 89% of patients in the liraglutide group and 46% in the control group.
Conclusion:
Significant evidence displayed that GLP-1RAs can be beneficial in treating PAD. They can reduce MALE, improve endothelial function, promote angiogenesis, and provide pleiotropic effects. It should be considered in patients with PAD. Future studies need more RCTs, females, and minorities to further investigate efficacy beyond white males.
